The managing director of a metal processing plant asked for support because regulatory inspections kept raising the same findings, and the internal safety organisation had struggled with the same problems for years.
The audit did not only list deficiencies. It also examined what authority and management support the safety specialist had. The person was professionally competent but had no means to overrule a production manager's decision.
The recommendation was therefore organisational: the safety manager reported directly to the managing director, a stop-work authority was established, and safety indicators were included in production managers' performance reviews. Repeated findings ceased in the next inspection cycle.
